The PM should try and resolve issues within the project and team, but sometimes a facilitator is needed when the PM can't resolve the matter.

I would involve HR and I did several times when I felt I needed expert advice because consequences of the badly handled conflict would be too significant. Skilled HR person is able to provide very good advice on how to handle the conflict in a constructive way. It also sends positive message from PM to the organization that he or she takes the PM job seriously.

It depends on the circumstance.

I agree it really depends on the situation/environment of the project.

Depends on project types, scope, infrastructure(s)/scenario(s) impacted etc some organisations have started to look at the possibility of a built-in ‘consequence management’ functionality (HR-related) as part of their major risk incidents management program.
