Issues...RAID Log

Is PM the sole responsible person to update or capture Risk Action Issue Decisions log?. OR stake holders can do this and.... PM is responsible for tracking and assessing the impact of those RAID items for project cost, time & scope and take corrective actions?.

The project team is generally responsible for capturing risks, but the PM is accountable.
As defined in the Project Charter and Project Plan

If you build the RAID log using SharePoint lists, the team can add to the list and the PM is notified of the update. So, you have the best of both worlds, collaboration and control. Also, these lists should be reviewed at the weekly team status meeting, so the PM can update any of the recent additions.

Always the RAID control and in its case, mitigation; accounts on the PMP, taking decisions on true results coming up from the process chart, and having analyzed deviation and rejections causes. After having applied a FME (Failure Modes and Effects) to be close to a diagnosis, which will be the basic paperwork for starting a "brain storm" among the project team...
