Project manager Kathryn is trying to attain her treasure of goal attainment. She does not have much time to spend in her analysis of previous projects' lessons learned, but she has concentrated on goal attainment in three areas: Activities/Phases/Processes, Teams and Individuals/Roles. She has a lessons learned report from an old project and has interviewed Frank.
She has completed her interview of Frank, who was unsuccessful in attaining goals in a project last year. Frank is now simply like one of those bodies on a pirate island pointing the way to the treasure. Kathryn does not want to end up that way.

Basically, Frank described his problems with surprises of new requirements at the very end of the development phase. He says his developers were adequately skilled.
Results of Review of Report
Her review of a lessons learned report from a similar project completed years previously shows that schedule goals were not attained during development phase then either.
The many survey respondents blamed an inadequate number of developers; insufficient time allowed for final deliverables from initial drafts; insufficient review time to get comments back to developers for final work.
There was discussion at the time over whether the skill level of the developers was adequate.
