Looking for an example (not a template) of an IT related project management plan. I am very new PM and am attempting to build a hardware and software upgrade plan for a small business; however, am having difficulties seeing a complete product with just a template. Parts of the plan looking/hoping to see:
- Purpose
- Project Objectives
- Scope
- Work Breakdown Structure
- Assumptions
- Risk Audit
- Organize the People: Roles and Responsibilities
- Keeping on Track: Monitoring and Control
- Communications Plan
- Evaluation Plan
- Closing Down
Any assistance with this would greatly be appreciated. Apologies if examples are listed somewhere on the site, I have searched with no luck. Saving Changes...

These are defined specifically to and within an organization. Past that, there is the consideration of IP and confidential information. The Purpose, Objectives, High-level scope, and Assumptions would be defined in the Charter. From there you can go continue to the WBS, etc.
You could take a communication plan from a template and adjust as needed. M&C is specific to the execution and project needs.

If your organization has a PMO you could request other PMs to provide the templates and project documents from past projects which can be used as reference.
Just as Andrew suggested some of the document info can be found in the Project Charter while others need to be created.
Like RACI for Roles and Responsibilities, an Org Chart based on people's hierarchy in the project and organization. You could use Gantt chart to create the schedule and worksheet to track the progress based on the schedule for monitoring.
You could also google these examples to get a high level idea and then tailor it based on your project need. There are few templates here in projectmanagement.com which can be used to start with under Templates section. Saving Changes...
I know many people think that by looking at an example project plan it will help them start the ball rolling, but actually it can also have a negative effect. You said it's a small business and a fairly common IT project (upgrade and rollout) right? The heading list you showed is OK, but you might be better of watering down (or using parts of) the PMBOK processes, and make it into something that matches the size and complexity of the project you are managing.
