RAID log needs to be maintained for multiple phases but depends on the project nature. If you are going to encounter same kind of issues / risks in consequent phases, it really makes sense to keep updating the RAID log for the whole project, which has many phases. I'd also suggest making it very easy for stakeholders to see what's meaningful to them - use a pull rather than a push method of sharing RAID info.
