Can one RAID log be maitained for multiple phases?

How can we best track Risks, Action items, dependencies for multiple phases for a project?

RAID log needs to be maintained for multiple phases but depends on the project nature. If you are going to encounter same kind of issues / risks in consequent phases, it really makes sense to keep updating the RAID log for the whole project, which has many phases. Make sure that you do a frequent review and keep updating it. Communicate & discuss with your stakeholders and maintain the log in your project repository tool.

Girija's answer pretty much covers it. Short answer, the same as for single phase. You could certainly add a flag to highlight phase origin if need be.

I'd also suggest making it very easy for stakeholders to see what's meaningful to them - use a pull rather than a push method of sharing RAID info.

As we have same stakeholders across all the phases having one RAID log with Phase origin sounds the best way to track.
