You might want to take it slow if you never have done lessons learned before. You can make it more complex after you get into the rhythm of performing more lessons learned.
1) Review the project with the team and ask everyone to provide what they feel worked and what they feel did not work. Keep the questions simple to bring out a discussion that will yield valuable input to the lessons learned document.
2) Create a simple excel spreadsheet to document your findings. Rank the findings with a 1-5 scale with 1 being very important.
Do not forget about that project lessons learned and file it away. Make an action plan to implement the discussed improvements on the next project.
