What is Witch-Hunt Meeting in Agile? How it is conducted? Has anyone done this?

I think that the Witch Hunt Meeting refers to the meeting that is made at the end of each sprint to talk about the lessons learn. What went well, what went wrong and how the team can improve next time.

A witch hunt is the opposite of what a retrospective is intended to be. The former focuses on blame whereas the latter focuses on learning.

I have never heard it called that. I have attended a few, didn't know that was the name for them.

I have never heard it called that. I have attended a few, didn't know that was the name for them.
