I'd suggest Googling for "PMP Application Assistant" - it will lead you to a site which will enable you to download a very useful Excel-format workbook which should help with the application process.

Some people spend many hours working on the application. It shouldn't take more than a few minutes after you gather the details needed. It took me less than 10 minutes.
