That sounds like two questions. The most common project management tool is probably Excel with Project a close second. The most effective tool will be the one that best fits your environment, culture and needs.

...does not always result in an effective tool. Tools that are highly customizable generally require high maintenance and have a high learning curve. If you get a tool that does everything from project to portfolio management, it can also be a lot of work to use in order to make sure you have all the data you need. It may be needed, and it may be worth it, I'm just pointing out the challenge.

I am using "Clarizen" PPM Tool which has loads of useful features to Support all kinds of project execution methodologies including Agile. Has support for Mobile platform and info sharing with non-members of the tool too. You would not require MS Project, Roadmap Tools & excel sheets for Graphs separately. Saving Changes...
