Lessons Learned in any tool?

I am looking for a software tool that will enable me to not only identify and manage risks and issues, but that will let me (preferably easily, but with work if necessary) turn resolved issues/risks into lessons learned that I can search from before starting a new project.

My goal is to capture lessons learned on all projects and then let PMs review them before setting out on a similar project.

So far, I have seen all kinds of products, but none seem to address this aspect. Any help would be greately appreciated.

Gordon,

We had a very simple process. Our issues were tracked in a Microsoft Access database (which, by the way, interfaced nicely with MS Project) and we were able to add a column in there towards the end of the project to record "lessons learned."

Re: Michael's suggestion -- there is an MS-Access Issues Management tool you can download from gantthead.com. This is not exactly what you are looking for, but could give you ideas.

An alternative would be to create a "docbase" repository, rather than use a database solution. This would be a collection of documents (Lessons Learned forms and/or other project docs) which you could use a free-text search against. This can become quite sophisticated, but does not need to be. If you use Microsoft's web server software, for example, it comes with a copy of Index Server -- you put all your docs in a single directory tree (in the simplest implementation) and you can query against it. Index is updated whenever a new document is added. More sophisticated document management tools and indexing tools exist, of course....

In our shop we use a tool from MERANT called PVCS tracker. WE use this tool to track issues, track software requests and use it for building tasks lists. It can be changed to just about any use and track any information that you would want to use it for.

PVCS (Merant) Tracker is difficult to install. The manual sends you all over the place... the merant knowledgebase doesn't provide relevant data - you need exact search terms and then you must compare versions. The manuals don't tell you anything. If it was up to me - i would return the software... I have NEVER been so frustrated in all my life.. it took 2 weeks to get installed, and how to integrate with Version Manager? Depends on which posting you find... I have tried 10. None make any sense or work correctly.

I would avoid this product at all costs...
it may do what you like.. but the installation
and the manuals are the absolute WORST!
