How do you ensure an efficient use of lessons learned?

How do you prevent that lessons learned gathered across a Project Management Department get tucked away in a register that no one looks at again?

Have you developed a process to close this knowledge management gap? What has been the outcome?

I guess this must be standard practice.

We prepare & present 'Lessons learned Deck' with Senior management & SQA as a part of Project closure exercise. Based on lessons & patterns, this goes as a feedback in the system to put controls in our processes or modify the templates & practices to arrest the problem in subsequent similar projects.

Also, as an additional step, during project kick-off, PM is asked to refer to check lessons learned for similar projects.
