how i can know if some organization are READY to use or transform to agile mindset, methodology and agile project management?

What common pitfall in that transformation ?

You can be sure about it when the senior managers have finally realized and write to you that their organisation's decrement in profits and loss of clients has something to do with the waterfall method they follow.
moreover, there are indicators. see if that company needs a product owner role or it is in need of a dedicated person for each product who can guide you about the requirements of that particular product. see if that company is ready to follow Scrum method and is ready to have dedicated teams that have members that are not pulled off the team until the project is complete. finally , Is your company ready to adapt Agile Mindset.
