IT Transformation Discovery Template

I just started at a new company and we're starting an IT transformation project. In the past, I've worked with large consulting services, (MS, Citrix, etc.) that used templates to make sure they asked all the right questions. It had all the basic minimum required info to support the changes with cues to help the consultant dive deep with the client.

Does anyone have something like that? Something I can use to evaluate a current system and have a high degree of certainty that I've got all the info needed to develop a suitable architecture in and plan to migrate to a new system.

like you I worked for a large consulting company, IBM, for 30+ years and saw and participated in many 'IT transformations'.
There is no general template I know of, there is a range of tools and techniques you may apply according to the situation.

The only big advice I can share is that it is not about IT, but always about business. In digitalization times, IT at best is embedded into business strategy, products and markets and capabilities.

I wouldn't expect to find any single template for IT transformation because that name could cover such a wide range of activities. Maybe you should put the question to the team(s) who will be doing the transformation.
