so based on the Professional business analysis guide, there was some scary statistics where it stays most of the projects fails because of failure in requirement management or developing a proper business case. and it suggest that to do it properly organizations should have a "Business Analysis Office" which correspond to "Project Management office"
my question is, is there any examples of real life companies who does have a business analysis office that takes PMI guide practice? Saving Changes...
Not so much an office, but I've worked with companies which have established Business Architecture and/or Business Analysis Centers of Excellence which provide support to BAs and BArchs across the organization.
I have not seen that name, however different businesses like to name their own departments in ways that reflect the priorities of their executives rather than use industry standard names.
Systems Engineering organizations includes systems architecture which deals with much of the same content as a BA such as requirements, and fitting a solution to the business need. They are more technically focused, but an effective product architecture must also meet the business needs. There may also be less technical and more business financial organizations that also cover many of the same types of information such as Business Management or something similar.
The responsibility may be split across different functions, or like PM roles you may have people performing that role in various functions rather than in one central location. Saving Changes...

I was in charge to create this type of offices in several companies along the years, including it my actual work place. In my actual work place, that role belongs to Strategy, Process and Transformation office. The reason is because Business Analyst is the critical role to help organizations to move from one state to other state (transformation) and that must be drive by strategy (strategy is the way organizations answer to environmental stimuli through the functions/process).
