Does someone have a template, of sorts, for a project kick-off call?

I have one ppt that is used as official template inside the organization I am working today. But the core is simple. Is about to explain (in no more than one ppt for each one, using "elevator speech" as much as possible):
-Why? reasons because the solution in terms of benefit. The definition is in terms of outcomes and goals.
-What? scope
-How? the way to create the scope
-When? milestones or some people put here the schedule
-Who? the team and the needs of engagement to the project
All these stuff is linked by the Critical Success Factors which are Be availble in the needed time, Be ready in the needed time, Be clear about the goal, Anticipate instead of react. Project Manager focus is to work in achieving the critical success factor

I'd suggest checking the templates section of this community, but you may also want to clarify which kick-off you are referring to as there could be different kick-off meetings held over the life of a single project.
