If you Google construction audit or project management audit you will find hundreds of commercial templates. It's a matter of finding one to suit your requirements. But first you have to determine what kind of audit you need. What concern or problem are you trying to solve - financial, quality management, communications, general performance, resource assignment, environmental, legal, compliance to corporate policy?
An audit is essentially a post-mortem of an activity. A review of what happened compared to what was planned to happen. How well did we carry out our plan. A construction audit may include contract compliance. A project management audit may include one, some or all project delivery elements.
I would normally lay out the project audit requirements when I develop the project plan rather than using someone else checklists or formats.
An other consideration is that an audit should be carried out by a arms-length party. The party that devised and implemented the plan should not undertake the audit.
My comments don't provide the specific information you requested but possibly provides some thoughts.

Thanks to all of you about commenting how to get Construction Audit Template to me, how to develop and lay out a construction audit template that suits my requirements. Having said this, as to me, there should be a guideline on how to prepare the template. Moreover, the rules for auditing must be strict with the specific law of a given entity or project or organization. I inquired a template because the auditing practices are stringent to fault finding sort of things in most cases. Sincerely, Engdaw Admasu

Engdaw -
Can you clarify whether you are leading a team to perform the audit, or will your project be audited?
Assuming it is the former, key things you'd want to identify in the template are:
1. Control objectives for the audit
2. Scope of the audit
3. Timing for the audit steps
4. Stakeholders involved from the audit team and audited organization
5. Any other rules of engagement related to how the audit team will work with the audited organization and how findings will be reported and responded to
6. Something to capture the findings themselves including categorization of the findings (e.g. severity)
