In my personal experience, no more difficult than other projects. I think this will not answer your question but is what I lived along the years. No more than that. So, believe me, it is impossible for me to differentiate grades of difficulty by type of domain. But again, just my personal experience.

You are welcome. The same than from others professionals in other domains. What helps me a lot is, before the initiative starts, perform elicitation activities about the domain where I will work. It does means understand the domain, understand the stakeholders in the domain, understand the process in the domain and understand environmental aspect of the domain. Putting it in other words, to understand the basic to be able to communicate with the stakeholders.

Respectfully, I fully disagree. The most difficult projects are projects where software component is part of the solution. Why? Because "the software intangibility". When you ask progress in a software component and somebody answer you "x %", how is she/he is measuring it?.

That is true given the fact that there are so many guidelines and rules the healthcare system has to follow. In the US, From HIPAA to CMS guidelines, there are lots of factors to be taken care of and on top of that there are federal as well as State level directives.
Another big reason is the data exchange between multiple systems within and outside the organization. So there is a ton of dependencies between the systems. A very simple example would be an upgrade project. Claims processed in a system is dependent on the Member data, Provider data, Plan data, and various other sets of data which might be residing in multiple different systems. The project will have to take into consideration all the impacts/ risks and dependencies while planning. The impact analysis of the changes in one application on the others need to be done. The test plan also need to take care of all the system as well as the regression tests. The data scenarios in healthcare are vast, so the data creation is a big activity which also need to take care of masking due to guidelines. And, of course the involvement of other related activities.
Overall the efforts are high and QA is comparatively more complex for a healthcare project than many other types of businesses.

The software solution and the measurement of the results depend on the clarity of the requests from the users or the client, how explicit they are formulated, and how measurable they are.
But I agree... sometimes the requirements are vague... and it becomes a challenge.
Healthcare projects might be more challenging than those done in unregulated industries, but the same could be said for military/aerospace, financial services or public sector projects.
Regulatory requirements, challenges with sustainment of changes from stakeholders and differing stakeholder agendas are common challenges in healthcare but these are also found in other regulated industries.

I have not seen yet a comparison of difficulties between industries regarding projects. In the end, someone said 'it is difficult before it's easy', so it depends on who looks at which projects.
My personal view is that specifics of HC that also affect projects are
- always humans are affected, many times with their lives and their health (hic!)
- it is an old industry full of tradition and hierarchy, inhibiting sometimes innovation
- it is vastly regulated, so it has a lot of bureaucracy and red tape, yet governments are loosing out vs corporations (e.g. US HC, NHS in UK)
- it is a money making, highly profitable industry and for-profits are greedy for more gains, and lots of power politics can be recognized (watch out for pharma giants and hospital providers), yet in crisis civil society steps in and tries to fill the gaps
