I’ve created a project risk/issues log and stakeholders have asked me to track general project questions and decisions in the risk/issues log. I’m not sure I should track questions in a risk log. Any thoughts Where I should track project questions and answers be tracked?

Something like a dedicated Confluence page or OneNote is pretty common, if you have a tool like these.

It could be structured as another worksheet in a RAID or risk/issues log, loosely structured in a Wiki page using a tool such as Confluence or unstructured such as a set of discussions in a Slack channel. It really depends on how the questions are being asked and how stakeholders want to be able to access the Q&A's.

Depending on the nature of the project, sometimes a published FAQ is a good idea, especially if the scope of the project or the approach being taken is likely to generate a common set of queries.
