Would recommend Jira. This tool is widely used by many of my/our clients as well as in our organization.

JIRA is a great tool, but it's counterpart, JIRA Portfolio, is specific to Agile Project Management. If your organization, or just the PMO, isn't fully Agile, I would recommend Smartsheet, Mavenlink, or LiquidPlanner as your primary Project Management software. All three integrate with JIRA, so your organization could have an internal Agile ticketing system integrated with the Project Management software.

Sanjay, did you ever decide on a tool?
Was there any particular reason the tool would need to be open source?

Before jumping headfirst into a tool, I consider the total cost of ownership. I've seen too many clients go for JIRA and then realize they need several plugins (for purchase) to get it closer to meeting their organizational needs.

I've been using Microsoft's Team Foundation Server with a very large organization for the last two years. They are using it successfully for agile portfolio management, program management, and project management. It's not cheap but it addresses the full stack, from code source to the portfolio.

Many will recommend VersionOne, Rally, JIRA, or AgileCraft. I've worked with all of them and they are all excellent tools.

If your still getting going you can always start out with the basics.

Whiteboard, 3x5 notecards for user stories and post-it notes... Don't forget to use your cellphone camera for the permanent record....
