Please help with an IT Project status template which includes high level milestones, in progress, completed and upcoming tasks with the project phase

It would be better to confirm with your key senior stakeholders what info they are looking for and how they would want it presented and then mockup a sample based on that and iterate on it through a few feedback and update cycles.

Then, as much as possible, set it up so that they can pull this info themselves rather than you having to expend effort on an ongoing basis in creating and pushing it to them.
