In my view, lessons learned is a project lifecycle activity. More often than not, lessons learned are performed at the end of the project (post mortem) and incorporated in the project end report. This report, let's admit it, will not likely be consulted and tucked away in a (digital) drawer.

Therefore, it is essential to promote and embrace a meaningful way to learn and improve throughout the whole project. This could be done during team meetings, similarly to the retrospective session held using scrum methodology.
