An excellent overview.

In my experience, the greatest challenge in Requirements Management is not capturing requirements.
It is preserving their connection to business intent as projects evolve.

Requirements rarely fail at the point of documentation.
They more often fail through gradual misalignment caused by changing priorities, evolving stakeholder expectations, new information, and accumulated decisions over time.

This is why traceability matters so much.
Its real purpose is not administrative control.
It is preserving the link between strategy, requirements, decisions, and delivered value.

Ultimately, effective Requirements Management is less about managing requirements and more about ensuring that projects continue solving the right problem as complexity and change increase.
