How do you manage the "middle layer" of project decisions?

Big strategic decisions get documented. Small day-to-day decisions usually don't matter.

But what about decisions like dropping a feature, changing a timeline, or choosing a vendor? They're important enough to matter later, but often end up buried in email, meetings, or document comments.

How do you capture these before they get re-debated weeks later?
