Does anyone know of any studies (or survey) as to what proportion of project risks can be attributed to poor quality?

I’m not aware of any research that attributes a specific percentage of overall project risk directly to poor quality. However, the impact of poor quality is well documented through increased rework, schedule delays, stakeholder dissatisfaction, and cost overruns. Project Management Institute (PMI) has published several articles on the Cost of Quality (CoQ), emphasizing that the costs associated with poor quality are often significant but difficult to quantify precisely across all projects. One useful reference is:

I’d also be interested to know whether anyone has come across an industry study that specifically quantifies the proportion of project risks attributable to poor quality.
