Team building is a continuous process.
Let me take the example of building the team in the initial stages of the project,more or less i will follow the same procedure when somebody joins in the middle.
1.I will introduce myself,my roles and responsibilities in the project.
2.Gives chance to all the team members to introduce themselves,their skills and ask them what is the unique thing they feel about the project for example in one of my projects for one team member it is a first project,for another one it is a first development project.
3.Explain the objectives/goals of the project and the benefits it is going to bring to the organization.
4.Set the expectations,ground rules and roles and responsibilities of each team member.
5.Trust is very important in team building and it is established by carefully understanding the strengths and weaknesses of each member and mentor them,provide constructive feedback to improve their strengths and to overcome weaknesses.
6.Understand their areas of interest to work on and try to assign that.
7.Provide a platform for open communication .
8.provide an opportunity for work life balance of course not at the cost of the project.
9.Always motivate them and recognize them for their good work.
10.send mails for their birthdays,important milestones for them for example completing 6 months,1 year in the project etc.
11.Spend some time with them talking about things other than project like their interests,hobbies etc and build a rapport. Saving Changes...

When the PM is taking over a project and especially a new project, it is important to know each project team member - their expectations, roles, responsibilities and how they see the project success.
The PM has to give them a feeling of ownership, foster open communication, mentor and provide ample opportunities for creativity within the scope of the project. Recognition and reward are two great tools to motivate the team.
AGILE project management is a great thing to build up the team. It gives them to self organize, collaborate and help each other. Saving Changes...

I normally follow the simple rule.... TALK with each team member daily.
1) When you talk you discover their problems (personal and professional), and when you solve their problems you get their support in project work.
2) Then make them work with each other in round robin fashion. This makes them to know each team member, else small groups will be formed within a team which is not good.
3) Take them all for snacks once in week, this will make them sit on one table and discuss their hurdles in open.
4) Teach them the benefit of creating good friends at work place. Saving Changes...
