Hello All,
Can you please share your experiences with respect to how you handled the project kick-off meeting and send me in private some project kick-off meeting templates.

have you checked the template database here at pm.com; as far as i can remember here are some kickoff tmeplates available.
If you will send me your email adress as a private message i am glad to send you another template which i mostly use.
The kickoff meeting has the purpose to bring the project team and (at least) the main stakeholders together (may be the first time) to gave them the possibillity to get to know each other. Main goal imho is to paint a clear picture to what the project is all about and to create a common consens about the project, its goals and objectives, the method and so on ...
For me it is NOT an operational project meeting.

I am a PMP aspirant. Why does the PMBOK not mention the Kickoff meeting anywhere? Although I understand Develop PT, we should not have to assume anything about project management. Some PMP trainers say a kickoff should happen after the baseline of the triple constraint, some say after all planning is finished and just prior to the execution phase and some say it could be done after every phase.

The PMBOK is not a check list of all you need to do. But you are right there is no mention on a kick-off meeting or start-up meeting, place for improvement. There is mention of a closeout.
In any case there should be a first meeting!

You're right. There is no any statement on the kick-off meeting in the PMBOK 5th Edition but It might be inclusive in the next edition of PMBOK. You can refer it to the PMP exam content outline for the purpose and timing of kick-off meeting.
All PMP trainers you mentioned above seem to have the same voice on timing of kickoff meeting since the baselines and plans together with Project Management Plan are key deliverables from the planning domain . And if project is divided into several phases, it can be held at each phase's planning domain dependent on the project situation. The timing of kick-off meeting can be practically decided based on size of project team, the project needs and situation. It can be held even in the executing domain as team building activities upon approval of Project Management Plan or it may be held just after Project Charter is approved.
But according to PMI's updated PMP exam content outline of Aug. 2015 (linked above), it is generally understood as a task to be done after stakeholders' acceptance on Project Management Plan in planning domain, considering the purpose and agenda of kick-off meeting. .

That is great to know. I plan on retaking the exam in Feb 17. I took the test two months after PMI approved my application and I was disappointed in my performance. I sure hope that it comes out after that time period. It took me 3 months just to fully understand all the ITTOs let alone working on everything else. I am definitely much more confident about the test at this point in my studies.

yes, the new PMBoK Guide is expected towards third Q3/2017 and the updated PMP exam will become valid in the beginning of 2018.
