I'm planning to come out with simple timesheet mechanism to ensure the team to record and see how effectively and efficiently they are using their time daily.

In this sheet, you can record how many hours per day per activity you spend and you can build on it if you're saying it is a good start. However, for my work I use a program called Harvest for time tracking and it is amazing, check it out as well. Saving Changes...

In fact Duration = Finish - Start, thus it is not necessary to put into the sheet.
To see how much work has done, you should seperate Finish as Estimate Finish and Actual Finish. For on going task/ item we need Percentage of done work. Therefore you can see the effective of your team. Saving Changes...
