I am the project manager for a small (12 odd) IT shop for a large non-profit (roughly 7,000).
Since we have several projects/initiatives underway, we are constantly doing surveys. Currently, our requirements surveys, and every other type is a fairly laborious process.
I would like some input from everyone on some reasonably priced survey software that is web-enabled and has a backend data store to allow for reporting, analysis, etc.

You can try Web Surveyor at www. websurveyor.com, we are using it here in another non-profit organization and it is quite simple and easy to setup. it costs 1,500 for a 12 month license.
Intuit also has a product that we use in our company. Very easy to use and relatively cheap. http://www.inquisite.com/ Saving Changes...
