Lessons Learnt Template

Does anyone has a template called "Lessons Learnt", you might have a different name, basically this will have all the details of the project from start to finish for lessons learnt. What went wrong, what problems were faced and how they were solved, which would enable next time to plan better and take preemptive measures to decrease the issues etc.

Definition of PRINCE2:-

"PRINCE2 is a process-based approach for project management providing an easily tailored, and scaleable method for the management of all types of projects. Each process is defined with its key inputs and outputs together with the specific objectives to be achieved and activities to be carried out.

The method describes how a project is divided into manageable stages enabling efficient control of resources and regular progress monitoring throughout the project. The various roles and responsibilities for managing a project are fully described and are adaptable to suit the size and complexity of the project, and the skills of the organisation. Project planning using PRINCE2 is product-based which means the project plans are focused on delivering results and are not simply about planning when the various activities on the project will be done.

A PRINCE2 project is driven by the project's business case which describes the organisation's justification, commitment and rationale for the deliverables or outcome. The business case is regularly reviewed during the project to ensure the business objectives, which often change during the lifecycle of the project, are still being met."

For those that are interested, there is no licence charge at all for using PRINCE2. The only cost is the cost of buying the PRINCE2 manuals which are available from the UK Government Stationery Office at a cost of approximately £55 each........

Dear Tarun, attached is a template that we use for Lessons Learned. It is somewhat high-level and intended to capture lessons learned information vis a vis processes, tools, performance, etc. We use the lessons learned document as an input to the PM's recomendation for PM continuous improvement. Some combine this into one process step and template, we find it helpful to have it be two to ensure that lessons learned are acted upon, rather than filed and forgotten. I can't remember where I pulled this information together from but this is a document that I used to pull together lessons learned from a recent project.
