I typically hold a RACI meeting on larger projects after I have held the project kickoff meeting. Within a project kickoff meeting I normally cover the following:
Introduce Official Sponsor(s) and Project Manager
Introduce Key Customers and Stakeholders
Review Project Scope, Definition and Objectives
Review High-level Timeline & Milestones, Roles, and Budget
Review Deliverables
Review Challenges
Explain Next Steps
However, I have spoken with other Project Managers who like to hold a RACI meeting with all stakeholders before the kick off meeting so that by the time the kick off meeting happens it is clear in everyone’s mind what their roles and responsibilities are.

We tend to define the RACI ahead of the project kickoff, and hold a pre-kickoff meeting with the stakeholders. Usually to the effect those members of the RACI develop material needed for the kickoff meeting. When you plan the project, and inside tha RACI chart, you do not have people names. You have roles names that you will transform into people names when you start the project execution. As soon as posible you have to start the negotiation to transform roles names into people names. So, in my experience, what you name "RACI meeting" is not needed. What is needed is to work with the leaders to negotiate the people names before the project execution and getting a strong agreement about those people will be assigned to activities when execution start. Normally, I create a RACI matrix for the core team and flush it out further as more key team members are added.
Usually, I have a kickoff meeting with the core team, stakeholders, client and vendor. Also, I create a website or SharePoint site with all pertinent documents. Your PMP will further delineate the roles and responsibilities as you move forward with your project. Saving Changes...
