What tips do you have for practitioners to help them recognize they are faced with a conflict of interest situation? What flags should they be looking for?

Agreed, however if there is an obligation to raise awareness (among parties to a contract for example) we will have to rely on the written word for guidance to remain consistent and in compliance. For example, I would be willing to provide my team direction in the areas of:
-Safeguards
-Approaches to Mitigation
-Policies
Documentation addressing the above can make a great asset to a project team.
