The acceptance criteria for each gate is defined at high level in Initiation and/or more detailed later in Planning. Elements of the acceptance criteria are typically key deliverables (e.g.training to end users) or milestones (e.g. Operations sign-off). They are key elements of the project plan. Also knowing as soon as possible the acceptance criteria for each gate really helps you planning, defining activities, identifying risks, identifying key stakeholders you might have missed... It is like going to an exam knowing beforehand the questions.
When you reach the next gate, you have to provide evidence to your Steering Committee in a more or less formal manner (depending on your Organisation) that you have meet the acceptance criteria to move to the next gate. In my projects (mostly SW related), some typical evidences of meeting the acceptance criteria are evidence of all test passed, sign-off by Operations (they are ready to support the project), sign-off by customer of a list of accepted deliverables...

PMO defines the stage gates. Dependent on the size/cost of the project, varying deliverables are required. Sponsor/Stakeholders/Steering Committee will decide on proceeding.
This is a good avenue to leverage for increased awareness in thee area of business realization. Meeting acceptance criteria alone does/should not automatically give a green light to proceed. Saving Changes...
