I did not participate due to the fact that I belief the answer format should be more of a "true or false", "yes or no", "agree to strongly agree", or "disagree to strongly disagree" responses rather than attempt to almost trick the participant. We do not need to choose "most correct" answers in a survey, this is more in line with the PMP exam format.

This is not a "true" survey but rather more of a "test" response format. Participants should not be concerned if they answered correctly for a survey.

I can go on but some really good information regarding surveys and various types are in the 6 Sigma protocols. I know a bit about this as I manage a specialized group of 6 sigma Champions for projects that meet those requirements outside or standard engineering projects at my organization.

Leaders and employees adhering to a code of ethics create an ethical organizational culture.The leaders of a business may create an ethical culture by exhibiting the type of behavior they'd like to see in employees. The organization can reinforce ethical behavior by rewarding employees who exhibit the values and integrity that coincides with the company code of ethics and disciplining those who make the wrong choices.
I am not the organiser of the survey, like I hope the result will be share.

Yes, I did and I heavily promoted it on my social network.

The more the results will be global, the better will be the survey's results.

I see Marcus' point, but I liked we were forced to chose among few scenarios, as we had to pick the one more suitable for our business life, even if we had a fifth option in our mind.

Agree that a fifth answer was often in my mind.
