Top 5 lessons learned for Business Case Development/Process Improvements

I am working on a new business case development for my organization. Based on experience what are the top 5 challenges/lessons learned that PM's or Business Analysts seem to encounter the most? If possible, I would like to develop a mitigation strategy for the business case if such challenges arise.

Here are the top 5 from my experience:
1. Poor definition and quantification of Objectives.

2. Poor communication Plan

3. Project Managers becoming Project Trackers and staying above the details and the content of the project. Too concerned with updating schedules than understanding progress, obstacles and interpersonal relatioinships.

4. No TEAM MIND / No TEAM EGO - the team needs to be single minded in purpose. Personal egos must give way to the TEAM's success.

5. No CEO / COO recurring demonstration of committment.

I liked your list a lot. I'm going to use it in a business plan I'm contributing to. One addition I'd make is Lack of clear organization, team, and personal priorities, and conflicting success models between members, teams, and organizations.
