What to do so that the lessons learned remain in the organization and are documented and do not go only in the knowledge of people?

I think that sometimes the lessons learned are not documented and only remain in the knowledge and experience of people who leave the companies also take with them the knowledge and that much is lacking in the future.

Before Project closure, call all the project stakeholders in a common place to discuss "What went right (WWR)" and "What went Wrong (WWW)" on a particular project.
Record the WWW & WWR, share with all team members and place the file in a centralized location.
When in future, if the similar type of project comes then refer the recorded WWW & WWR.

People compliant about knowledge gap and taking away, it is just an excuse
If we have right people and experience they can handle things , only people take away the history

Progressively capturing lessons learned under the PM knowledge areas helps a lot. Also makes it easier to search though you may have to add some headings for engineering, safety etc.
Good excel sheet with required information fields is really all you require. Discipline in doing it regularly is key and getting everyone's input should also be considered. You will be surprised by the good lessons you get from what may seem as difficult stakeholders.
