What are the best practices to improve the efficiency of developers and leaders?

Mainly in any organization, management always look forward that developers/leaders should improve their efficiency. e.g if any developer who has previously taken 8 hrs to complete the task then over the time period of 6months or a year, s(he) could deliver similar tasks in maximum 5-6 hrs. But in many cases it didn't happen and after a year management has to appraise them as well.
So in nutshell, how to improve the efficiency and overcome on such issues?

I guess that interesting projects and challenges are important factors. People won't be efficient and put effort in something that is boring, especially when they need to manage 10 the same projects one after another with nothing exciting going on. The motivation and performance will stay at the same low level or even go lower.

Sorry Prashant but your answer is not correct as you have stated it (sorry if I missunderstood).
Or perhpas I undersood that because the example you used And please let me say that I do not agree with some of the comments above. Effciency is an important factor to define productivity. In the case of software (and what it was extended to other industries) IBM make a lot of studies on that field. The first one was in 1970 (you can find it into the internet) and from that time when you search the same studies but updated there are not sustancial changes. You will surprise about what helps in productivity and efficiency. For example, the square inch of work space available to each person. So, search for those studies that will help you a lot.

You've received several good answers about what you can do, but has the question been asked whether improved efficiency is possible, or if you are reaching/exceeding the point of diminishing returns? Was the goal defined by management, or by an expert who knows what it actually takes to complete the work? If the latter, you could apply most of the answers you have been given and see results. If the former, it's possible, but could be less likely.
