LL should be recorded through out the project phase and the template should be accessible to the stake holders for recording the same . Review of those can happen at every gate review to add/modify/delete to finalize LL

To Lori's point, capturing lessons learned should be an on going activity throughout the life of a project. All team members should be encouraged to capture the lessons learned as they become aware of it.

To me lessons learned are risks the project team failed to identify. A review after each PLC phase is a great time to discuss lessons learned and there by potentially mitigating the risk should it occur in the next phase.

I believe it depends on your environment such as the size of the organization, project, team, culture, ... ets.
you need to understand your needs and capabilities to design your own BEST PROCESS that works for your firm.

I personally take notes throughout the project saving dates, issues I came across and how these were resolved and what were the consequences.

At the end of the project I would meet with my team and go through them, receiving feedback and opening discussion from which we build the bases of improvement for the next project.

1. Collect them frequently, not just at the end of a phase or project.
2. Categorize them and treat them appropriately. Reminders need to be handled different than blockers which need to be handed different from true knowledge.
3. Don't put them somewhere where no one will find them - curate and organize them so that they are easy to locate.
4. Massage, scrub and add context to them so that they are truly useful.
5. Best of all, bake them into your SOPs & standards so that folks don't have to look for them!

It really revolves around three steps
* Capturing feedback/evaluation, preferably anonymously
* Identifying lessons learned from the feedback/evaluation
* Incorporating lessons learning into the organization process assets

Without the third step, you may as well not do the first two.

I think you determine how to capture as you're going through the project. You definitely should have a scheduled meeting during project closeout, but I would capture feedback along the way too.

LL is a continuous process. It need to be done on a regular basis to catch information before the project team is dismantle.
Other previous comment are also key, share otherwise no point to do it.
