A very good question. As our brain filters information and how we perceive/ interpret certain human behavior, decisions etc. in a project leads to cognitive biases we cannot fully avoid. In order to at least reduce the impact on cognitive biases in a Lessons Learned Sessions (Looking Back and Looking Forward) a "safe environment" for the team members is important to allow for 360 degree views, let people speak out without fear/ group pressure and to have an outside moderator who drives the discussion from different perspectives.
