project management has two facets: process related and person related. To get your team and/or stakeholders to really open up, you have to focus on the collaborative side where it will be known to all that nobody will be penalized for stating their opinion. It's tricky but, can be very beneficial to ensure continuous improvement. Personally, I had great lessons learned sessions inclusive of my team and clients and business partners were everyobe collaborates for long term benefits.
