How do you deal with project operational issues?

As a project manager, we all have to deal with operational issues during the project life cycle. Often time these issues can be escalated very easily, so we need to be very sensitive in terms of managing them. We also know that it's impossible to prevent certain things from happening, but we could act faster to let the clients or key stakeholders know and seek alternatives. I think proactively setting up the decision making structure, including where all the key stakeholders fit in, is very critical in managing operational issues. What is your opinion?

I think those things are inevitable but to minimise or avoid them you need to ensure a very solid effective communication plan. Communication is Key when it comes to such operational issues.

To build on Rami's feedback, discussing things like how operational issues are to be handled, who to escalate them to, and so on early in the life of a project when meeting with key stakeholders can help to avoid surprises down the line.

This goes hand-in-hand with working through roles & responsibilities so that the project manager and team don't take ownership for things outside of their jurisdiction.

I agree that it's important to let the team members know, up front, who is responsible for what and clearly lay out expectations.

A RACI chart is good for this, and include key decisions as assumptions.
