How do you deal with project operational issues?

As a project manager, we all have to deal with operational issues during the project life cycle. Often time these issues can be escalated very easily, so we need to be very sensitive in terms of managing them. We also know that it's impossible to prevent certain things from happening, but we could act faster to let the clients or key stakeholders know and seek alternatives. I think proactively setting up the decision making structure, including where all the key stakeholders fit in, is very critical in managing operational issues. What is your opinion?
