Does anyone have a good base template that can assist with transition to Operations after the project completion?

Many projects end with a transition to Operations of an organization. I do not see templates in regards to properly documenting this transition. There are a number of areas that need to be covered and detailed, that I am sure are similar across most industries. I am not talking about going back to lessons learned to see where operations needs to improve upon. I am talking about documenting how Operations continues to carry on what the project has set up and successfully implemented and moving it forward to daily operations with the organization. Does anyone have a good base template that can assist with transition to Operations after the project completion?

I'm interested in this, too. We have a number of endeavours that start off as a project to get them off the ground, but then they become operational and the support process changes. Typically, the project team will have supported it during the project phase, but then the project team disbands and the support is left to helpdesk folks.

I'd be game for collaborating on something to this extend if there is other interest...

Ideally this transition should happen as part of the scope of the project and be a pre-condition for closure of the project. That should include the operations teams staffing up to support the change, knowledge transfer, transition of deliverables and so on.

This is very context specific - for example, transitioning support for a developed software application is very different than transitioning a business process.

Agree with Kiron.
In my view Operations Management procedures including transition steps/activities should kick in and overlap with Project closure.

Personally, I've never seen a base template for this purpose. My own experience is that the transition is very specific to an organization which would make it difficult to template. Maybe it would be better to define best practices for this transition? I would normally look at two areas - the overall objective/s of the project and the operational model of the organization. Based on this I would identify the KPA's that needs to be satisfied to successfully implement the deliverable into the operational structure. The KPA's would be defined with operational input.

Renea,
Transition to Operations is very much dependent on the business, scope, organisation & support agreements. It should be planned even prior to project closure. Development & Operation teams should work together and prepare the knowledge transition documents comprising of complete system details of the applications to be supported, infrastructure, stakeholders, third-party tools/vendors and all.

I can not share the template because strategical reasons but perhaps the following comment could help a little. Role on charge of that is the business analyst. In my actual work place we have a governance model where "final preparation" and "go live and support" steps must be performed before "project close". Both steps are intended to transition the product/service/result created by the project. In the same governance model at the very begining we have "project preparation" step where project requirements are derived from product requirements mainly all needed to transition the project results (training for example). The dynamic of the execution of the governance steps is determined by the project life cycle. Into each governance step we have deliverable to create.
