Does anyone have a good base template that can assist with transition to Operations after the project completion?

Many projects end with a transition to Operations of an organization. I do not see templates in regards to properly documenting this transition. There are a number of areas that need to be covered and detailed, that I am sure are similar across most industries. I am not talking about going back to lessons learned to see where operations needs to improve upon. I am talking about documenting how Operations continues to carry on what the project has set up and successfully implemented and moving it forward to daily operations with the organization. Does anyone have a good base template that can assist with transition to Operations after the project completion?

I have, but they vary significantly depending on many factors such as the system being transitioned, the level of criticality, and the delivery approach. For example if you're phasing in software incrementally, you have more opportunity for post-implementation changes than if you're launching a satellite into orbit. For a more detailed explanation, I would suggest searching the SEBoK (Systems Engineering Body of Konwledge).

A good starter for this is the PMI Standard for program management. In the program closure phase its all about transitions (yes, there are many).
I did several transitions, in ERP, outsourcing, in any client acceptance. Also, Organizational Change management is part of most transitions. Important is to involve future owners/users of my project product early, plan ahead, they will determine the overall success of your project.
