I agree with others that you may not need a PMO. If a PMO is going to succeed it will need to have buy from everyone. If its sole purpose is to police project managers you will be met with a lot of resistance.
Whatever you set up as a solution, I would suggest not calling it a PMO.
Maybe what you really need is a Risk Manager or a Risk Oversight Group. Focus on what the true issue you are trying to solve. If it is to track risks, then narrow down the Risk Management process for your company and work closely with the project managers to adopt it. Saving Changes...

At this time, if the project management depend only on PM may have risks. If "good PM", all projects is ok and vice versa. So we need another team/person who can monitor some important projects to make sure risk/issue from projects will be identified and solved soon. Moreover, that person/team can share project management experience, best practices... between projects Saving Changes...

PMO can be setup if there are more than one project, setting up PMO will cost the company.

You can treat as a project and to but the project charter for that request describing all the requirements , purpose , goals , tools , etc Saving Changes...
