Yes, you should be responsible for your own mistakes without trying to find an escape goat.
Accountability is the key characteristics of a project manager. PM is fully accountable for any misstep taken in the project. You must own the responsibility and resolve the issues.

I learned early on in my career to address issues, not people. When we address issues, we can redirect the blame game. I also learned that if I messed up, I can apologize for the issue, and outline what I learned from it, and assure those impacted that I will work hard not to let it happen again, the issue is viewed as a positive by my leaders and the project team. If I try to make excuses, it will make the issue drag on and hurt me.

Rightly said, Doing mistake makes you human and accepting them a true human. With your acceptance, your chances of refining increases and your conscious will save you from repeating that mistake.
