Workgroup Workflow Planner

The form can be completed by the workgroup working independently, or as a formal intervention initiated and facilitated by the project manager (or designee). Go through the workflow components in order. Check each box in the left-hand column that applies for a component. Then add a detailed description in the right-hand column. Add new items on the left column as appropriate. After all the components in the first table have been completed, complete the second table by discussing action items necessary to eliminate the workgroup workflow problems identified.
