Cause & Effect Diagram

  1. Create a “fishbone” diagram that can be projected or viewed on a large scale.
  2. Write the situation/problem statement. It should be precisely stated and clear to everyone in the group.
  3. Determine the cause categories.
  4. Brainstorm all causes.
  5. Once responses have been exhausted, evaluate the diagram for “root causes,” which are the initial problems that create others on the diagram.
  6. Assign next steps.
