Specifications/Procedures
- Create a “fishbone” diagram that can be projected or viewed on a large scale.
- Write the situation/problem statement. It should be precisely stated and clear to everyone in the group.
- Determine the cause categories.
- Brainstorm all causes.
- Once responses have been exhausted, evaluate the diagram for “root causes,” which are the initial problems that create others on the diagram.
- Assign next steps.
