This is a risk management template that includes alternate/backup risk management plans as well a risk score based upon probability and impact. Usage is simple--just fill in the columns. When you enter values for probability and impact, you must enter values between 0 and 1. A risk score is calculated automatically. You can rank manually, or sort by score and then rank. 

Data entry is simplified with drop-down values for Category, Strategy and Owners. You should always assign a singe risk owner. If you need to change the values, go to the specific tab and change or add new values. They use named ranges, so make sure you add the new entries within the specified range.
