In few words in your experience why do you think some project managers failed or mishandle a project?

Project Sponsorship Responsibilities

One of the trades of a successful leader is to grasp the importance of responsible sponsorship. A responsible sponsor not only seeks business success but also strive for added value to the community.

International academic studies show that many major projects fail because of poor sponsorship. As a leader, you often have to deal with sponsors yourself, for instance in the case of you leading a sponsored project. On the other hand, you are also a kind of sponsor to your team, your tribe. Maybe more than any other stakeholder, as a sponsor, you must be fully aware of your WHY, your purpose. That will give you the edge to be credible and trustworthy in getting your message across.

The studies show the perceived prime sponsorship responsibilities to be:
• Purpose
• Goals
• Conditions
• Engagement
• Governance
• Redemption

Yes,
Tamer mentioned most of the reasons of why project managers are failed.
Addition to that:-

The projects are not aligned to business and strategy, no business values,Absence of leadership skills,Absence of management support (key stakeholder like sponsor).
and remember the English Idiom which saying
"go BIG or go Home" = the statement which mentioned by Kiron.

But to be honest whats considered a project failure? that it went over budget?, that it never finished? and to be honest in certain types of projects failure in certain phases is expected in order to jump to the next phase.

I have failed when I expected people to do what they told me they were going to do and I didn't follow up. 9 times out of 10 it is someone not on the project team, but a stakeholder that needs to do something for us to move the project along. They have other duties and getting things done for the project isn't always their first priority. In some cases, they don't really want the project to go forward. So my not following up and just expecting them to do their part has forced me to reevaluate always being the helpful servant leader. Sometimes you got to be the guy who sends daily high priority emails to someone while cc'ing their boss and boss's boss.

By the way thanks for your time, and let me ask you a question then, do you think that perhaps by sending task reminders in a form of an email (daily) in this case to "the stakeholder" would have reduced the impact?..

Interesting answer, and it seems to match few other answers, it seems that stakeholders are or could be defined as a "risk" :)

So many possible answers to this question! I'll limit my answers to factors that are within the control of the PM.

The two that come to mind:

1. Lack of understanding of how to do their job particularly for the environment that that are in.
2. Box checking i.e. completing the "required" deliverables but not actually delivering any value.
