Many reasons like: lack of experience, Not follow ethical and responsibilities tips, Not follow communication management procedures, lack of PM skills, Not all stakeholders were identified, lack of management knowledge plan, not follow information management system, not all requirements were collected and its very important to create WBS and estimate cost and time to prepare baselines,...etc.

Addition to that:-
The projects are not aligned to business and strategy, no business values,Absence of leadership skills,Absence of management support (key stakeholder like sponsor).
and remember the English Idiom which saying
"go BIG or go Home" = the statement which mentioned by Kiron.

But to be honest whats considered a project failure? that it went over budget?, that it never finished? and to be honest in certain types of projects failure in certain phases is expected in order to jump to the next phase.

I have failed when I expected people to do what they told me they were going to do and I didn't follow up. 9 times out of 10 it is someone not on the project team, but a stakeholder that needs to do something for us to move the project along. They have other duties and getting things done for the project isn't always their first priority. In some cases, they don't really want the project to go forward. So my not following up and just expecting them to do their part has forced me to reevaluate always being the helpful servant leader. Sometimes you got to be the guy who sends daily high priority emails to someone while cc'ing their boss and boss's boss.
